ASSESSMENTS
Screenings include initial screening and triage, Comprehensive Clinical Assessments, and Psychiatric Evaluations that are done on-site by trained and qualified staff.
OUTPATIENT THERAPY
Outpatient psychiatric therapy sessions can be coordinated for adults and child/adolescent patients; intensity varies depending upon individual needs.
MEDICATION MANAGEMENT
Medication Management can be coordinated for individuals prescribed or needing psychiatric medications and/or evaluations.
COMMUNITY SUPPORT TEAM
Support Team (CST) services are services and supports necessary to assist adults ages 18 and older in achieving rehabilitative and recovery goals. This is an intensive community rehabilitation service that provides treatment and restorative interventions to assist individuals to gain access to necessary services, reduce psychiatric symptoms and develop optimal community living skills. The Community Support Team (CST) will provide assistance and support for the individuals in crisis situations, psycho-education and support for the individuals and their families, individual restorative interventions for development of interpersonal, community coping and independent living skills, development of symptom monitoring and management skills, monitoring of medication and self-medication and shall be documented in the consumer’s Person Centered Plan.
PSYCHOSOCIAL REHABILITATION
Psychosocial Rehabilitation (PSR) is a service designed to assist adults with psychiatric disabilities increase their functioning so that they can be successful and satisfied in the environments of their choice with the least amount of ongoing professional intervention. PSR focuses on skill and resource development related to life in the community and to increasing the participant’s ability to live as independently as possible; to manage their illness and their lives with as little professional intervention as possible; and to participate in community opportunities related to functional, social, educational and vocational goals.
Psychosocial Rehabilitation is based
on the principles of recovery, including equipping consumers with skills, emphasizing self-determination, using natural and
community supports, providing individualized intervention, emphasizing employment, emphasizing the “here and now”, providing early intervention, providing a caring environment,
practicing dignity and respect, promoting consumer choice and involvement in the process, emphasizing functioning and support in real world environments, and allowing time for interventions to have
an effect over the long term.
